When you have healthy employees, your company reaps the benefits. A correctly designed wellness program can increase productivity, boost morale and reduce stress.

Wellness Incentive Plan

Biometric results from an examination or screening conducted anytime between June 1st and October 31st 2023 will count as eligible for the Deductible Period for the following calendar year.

The Mayfield Medical Benefit Plan has a baseline deductible of $1,000/single or $2,000/family per calendar year.

All employees can earn credits to lower the deductible by (a) meeting the biometric target, or (b) submitting a physician’s certification form with a medical professional’s certification that you are being treated for the condition being evaluated.

Individuals have six (6) opportunities to earn a maximum of five (5) potential deductible credits for a $150 deduction per eligible credit met.

Employees who opt not to participate in the voluntary onsite or personal physician direct biometric screening shall pay a premium surcharge of $20 per month for single and $40 per month for family coverage. Such surcharge shall be paid in addition to the standard employee contributions.

New Hires enrolling in the plan mid-year, or those with a qualifying life event, will have the opportunity to earn the wellness credits by completing their visit and biometrics through an onsite clinic appointment.

Wellness Reimbursement Program | Administered by Davevic!

Effective April 1st, 2023, Davevic Benefit Consultants will administer Mayfield’s Wellness Reimbursement Program! There is no change to the program, Mayfield employees are still eligible for up to $90 per quarter on qualified wellness expenses.

Reimbursements submitted to the Wildcat inbox as of 3/31 have been submitted to Davevic on your behalf!  Any additional reimbursements for Q1 need to be submitted to Davevic directly. Instructions to submit online or via the mobile app can be found in the Wellness Reimbursement Packet attached! Quarter 1 reimbursements are due by 4/15!

  • All requests for reimbursement, supported by receipts, must be submitted along with the Wellness Reimbursement Form to Davevic at [email protected] by the 15th day in the month following the closed calendar quarter. For example, forms and receipts for the 1st quarter (January, February, March) are due by April 15th, checks will be cut the week of 4/17.
  • Incomplete forms will not be accepted nor processed. Please ensure that your email includes dated, legible receipt(s) and a dated, signed, legible form. Please include all attachments in one email.
  • Through Davevic you can submit your Wellness Form and Receipts through the Davevic Consumer Portal or Davevic Mobile App. Please read through the attached Wellness Reimbursement Packet, under general resources for step-by-step directions.
  • Instructions on how to access or register your account, file reimbursements/claims, and instructions to set up your direct deposit can all be found in the Wellness Reimbursement Packet, under general resources.
  • When submitting electronically, you must still attach a copy of the Wellness Reimbursement Form, for the claim to be adjudicated.  Form can be found under general resources

  • Please note, if you are already active in Mayfield’s Wellness Reimbursement program there is no action required.

  • Blood Pressure at 140/90 or below
  • Blood Sugar at 100 or below or 6% a1c or below

We are committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Wellness Reimbursement

To encourage practices that promote health and well-being for our employees, Mayfield City Schools will reimburse costs associated with wellness pursuits as outlined below:

  • Employees send completed wellness reimbursement form to the Administrator, Oswald Companies, by the 15th of the month following the close of the quarter.
  • Oswald Companies approves expenses according to medical carrier funding guidelines.
  • A separate third-party administrator, North Coast Administrators, processes payments and mails directly to the employee.
  • Employees should expect to receive their check within four weeks of the check mail date.
  • Direct deposit is also available. Please fill out the attached direct deposit form if interested.
